Case Manager

Employed and self-employed options available
Full or part-time (min. 3 days per week)

As a Case Manager, you will play a pivotal role in coordinating and managing comprehensive rehabilitation and support packages for clients with complex needs, including brain injuries, spinal cord injuries, amputations, and other catastrophic injuries. You will work closely with clients, their families, and a multidisciplinary team to ensure that all aspects of their rehabilitation and support are effectively managed. The role involves working within a variety of funding streams and alongside the legal process.

Client Assessment: Conduct thorough assessments of clients’ needs, considering their pre-injury circumstances and post-injury requirements.
Coordination: Instruct, lead, and monitor a multidisciplinary team of professionals to deliver rehabilitation and support services.
Advocacy: Act as an advocate for clients and their families, ensuring their voices are heard and their needs are met.
Goal Setting: Establish and review client-centred goals, facilitating independence and enhancing quality of life.
Resource Management: Liaise with relevant agencies to secure funding, equipment, and services necessary for client care.
Support and Training: Provide training and support to support/care teams, ensuring high standards of care and safety.
Care Planning (depending on the client’s needs): Develop and implement personalised care plans tailored to each client’s unique needs and goals.
Documentation: Maintain accurate and detailed records in the form of case notes, reports, assessments and cost estimates.

Qualifications and Skills

Education: Bachelor’s degree or equivalent in a healthcare-related discipline.
Experience: Registered with the Health & Care Professions Council or Nursing & Midwifery Council.
Experience: Minimum of 5 years of professional experience in a specialist rehabilitation setting or social care, managing complex cases.
Skills: Excellent interpersonal, verbal, and written communication skills. Strong decision-making and active listening abilities. Ability to problem solve and work sensitively with vulnerable clients.
Attributes: Self-motivated, committed, and enthusiastic about improving clients’ lives. A collaborative and proactive approach to case management.
